What are school-based health clinics?

School-based health clinics work to improve the health of students, increase access to healthcare, and decrease time lost from the classroom. It is a significant part of our organization’s outreach so that our youth stay healthy and ready to learn.

Who is eligible for clinic services?

All students at Hayward, Hawthorne, and Terry Redlin Elementary. Staff and community are welcome at these locations as well.

What services can be provided?

  • Routine physical exams—including sports physicals.
  • Diagnosis and treatment of illness and minor injuries.
  • Vision, dental, and blood pressure screenings.
  • Immunizations.
  • Health and nutrition education and counseling.
  • Medical and mental health management of chronic conditions such as diabetes, asthma, ADHD, etc.
  • Behavioral health services including screening, assessment, and medication management.
  • Complete dental services, including treatment (with additional consent).

Can the school clinic provider serve as my primary provider?

Yes! The medical and dental providers working in the school clinics are well trained and able to serve as your medical and dental home.

What are the payment options?

FCH accepts most insurance, including Medicaid and Medicare. We also offer a sliding fee discount based on your family size and income. Contact our billing department at 605-367-8770 for more details.

Will signing up mean that we can’t use our family doctor or dentist?

Our school-based health center is not meant to replace your child’s provider or dentist. However, you can still use our services even if you have a provider. You may find it’s convenient for your child to receive care right at the school if they get sick or injured at school. The school and our clinic work as a team to help children avoid missing school and manage care.

Can I choose whether my child will receive services?

Yes. The clinic will contact you prior to any treatment your child receives. You can be present at the time of the visit, but it is not required.

Will my child’s information be kept confidential?

Yes. By signing the consent form, you give the health clinic staff and school nurse permission to communicate and share medical information regarding your child’s medical condition on a need basis, with the understanding that this information will be treated in a confidential manner.
