What is a community health assessment?

Every three years, the City Health Department works with Avera, Sanford and the Sioux Falls VA Health Care System to take stock of the community’s health. We survey residents, hold focus groups, and review health data from local, state and national sources. Together, these give us a full picture of the community's current health needs, strengths, and opportunities that help shape our public health priorities and focus for the next three years.

Why your answers matter

Health records show who came in for care, but they don't show who couldn't get an appointment, who couldn't afford care, or who didn't know where to go to seek treatment. Taking the Community Health Assessment Survey is how we can hear directly from you on the needs that aren't always obvious through health records alone.

Our 2025 Community Health Assessment is how oral health became a priority for our community. Residents that completed the survey shared experiences of not being able to receive dental care - an issue that would have never shown up by simply looking at the charts.

This survey is not only an assessment of services at Falls Community Health or the Sioux Falls VA. This survey also informs us on the care that patients receive at Avera and Sanford so that we can gather a full picture of our public healthcare network in Sioux Falls.

How to participate

Take the survey online by Monday, Nov. 30. (Translations are available.)
Fill out a paper version at Falls Community Health.
