Falls Community Health can provide various services and support for individuals who are homeless. Patient care, education, and support are available within and outside the clinic. In addition to medical assistance, patients can be connected for help with other needs.

When it comes to determining who might benefit from the outreach program, the following indicators serve as signs:
  • Complex health issues,
  • ADL (Activities of Daily Living) concerns
  • Referral to medical specialists
  • Case management to assure needed care
  • Coordination of patient care by multiple health care providers
  • Medication issues
  • Patient education
  • Follow-up care
  • Dental/vision
  • Housing
  • Drug addictions/dependency
  • Employment
  • Education
  • Finances
  • Food/clothing
  • Legal

  • Dental Clinic voucher
  • Transportation
